reports to; director of clinical services

qualifications:

  • msw from a graduate school of social work and licensed by the state of new york (lmsw)

  • must have at least two years of supervised experience in the health field or two years experience in family therapy/counseling

  • must receive certification in hospice and palliative care within three year of hire date and maintain certification during employment

  • ability to relate to a diverse population and workforce in a personable and effective manner

  • ability to relate to the terminally ill and their families

  • must have home internet and email access and be proficient in microsoft windows

  • must possess car, valid driver's license, and current insurance

    physical requirements:

  • physical agility including but not limited to: walking, bending, stooping, climbing stairs, sitting and speaking on phone for long periods

  • periodically lifting up to 25lbs.

  • vision acuity correctable to normal

  • hearing acuity correctable to normal

  • ability to verbalize so that the average person can comprehend

  • ability to write, perform data entry and look at computer up to several hours daily

  • cognitive requirements:

  • actively participate in coordination of all aspects of patient’s care, in accordance with current professional standards and practice

  • develops and evaluates the poc

  • ability to provide direct care according to the established patient plan of care and delegate to others

    responsibilities:

  • contributes to patient and family counseling and education

  • participates in ongoing interdisciplinary comprehensive assessment

  • assesses patient/family psychosocial response to the terminal illness and evaluate coping skills

  • assists patient, family and agency staff in utilizing community resources

  • assists team members in understanding the significant social and emotional factors related to the care of the terminally ill patient and family

  • coordinates with team members to plan and implement the hospice plan of care

  • participates in interdisciplinary team meetings

  • facilitates group support sessions as appropriate

  • maintains accurate, thorough and professional written records, and charts relevant information in a timely fashion, following uh documentation standards

  • participates and is actively involved with the bereavement portion of the program as appropriate

  • participates in volunteer and staff education programs

  • develops and implements the discharge plan in conjunction with the ids

  • participates in qapi program and hospice sponsored in-service training

  • maintains point of service documentation at a minimum rate of 80%

  • accepts other responsibilities as assigned

  • refers high risk patients

  • develops/maintains community resource file

  • supervises msw students

  • accept additional responsibilities as assigned
